Statistisch signifikante Korrelationen zwischen bestimmten HLATypen und FA sprechen für eine genetische Veranlagung. Die systemische Provokationstestung ist der Goldstandard in der Diagnose. Die topische Provokation an den zuvor betroffenen Hautarealen kommt als eine alternative diagnostische Methode in Betracht. Sie ist aber noch nicht standardisiert und variiert für verschiedene Substanzen. Das Geheimnis der Lokalisationspräferenz in FA bleibt noch ungelöst, wobei die lokalisationsbestimmenden Rollen von gewissen Medikamenten,Traumaherden und viszerokutanen Reflexgebieten diskutiert werden.
