Leukaemia cutis (LC) is a specific extramedullary skin manifestation that can occur in various types of leukaemia. The clinical spectrum of LC is broad, and skin lesions most frequently appear as solitary or disseminated macules, papules, nodules or plaques. We present an unusual case of acute myeloid leukaemia (AML) with cutaneous infiltrates, which developed in a traumatically induced leg ulcer in a patient with venous insufficiency. An 81‐year‐old man presented with an 8‐week history of a painful, irregularly shaped, purple ulceration located at his right medial malleolus (Fig. ), which had been induced by local trauma. The patient's general health was poor, with recurrent episodes of fever and weight loss of 6 kg over the past 3 months. He reported a history of venous insufficiency, and several years previously, he had developed a venous ulcer on the other leg.Clinical findings at first presentation in our department. An irregularly shaped, purple, superficial ulcer, 20–50 mm in diameter, with grey‐yellowish exudation was present at the patient's right medial malleolus.Bacterial cultures of the lesion on the right leg revealed colonization with Pseudomonas aeruginosa.Laboratory examinations revealed increased serum lactate dehydrogenase (359 IU/L; normal range 10–250 IU/L), monocytosis (76%; 2–8%), anaemia (haemoglobin 7.5 g/dL; 13.0–18.0
