Maxwell, FRCSEd, FRCR, and Hari Mamtora, MD, FRCR Epidermoid cysts are rare benign tumors of the of testis, accounting for less than 1% all testicular tumors.â Although their clinical and pathological features have been well described in the literature, little is known about their ultrasound appearance. We describe a case of epidermoid cyst that showed ultrasonic features atypical of malignant testicular tumor and which correlated well with the pathological appearance of the lesion. Recognition of the benign nature of this lesion on ultrasonography may prevent an unnecessary orchidectomy. CASE REPORT A 19-year-old man presented with a three-month history of a lump in the left testicle, which had recently been causing some discomfort. He was otherwise well and denied any urinary symptoms. He had previously undergone surgery at the age of 12 years for suspected torsion of the testis but had no other significant previous history. On examination he appeared generally well. A hard lump was palpable in the upper pole of the left testis, which was clinically considered to be a malignant tumor. The patient was referred for an ultrasound examination of the scrotum (Figure 1).This showed the presence of a well-circumscribed solid mass measuring 1.3
