Hydrocolpos in an 18‐year‐old man with mixed gonadal dysgenesis Menashe Barzilai, MD Hydrocolpos is defined as a cystic dilatation of the vagina resulting from distal obstruction due to congenital anomalies such as vaginal atresia, a transverse vaginal septum, imperforate hymen, a urogenital sinus, and cloaca1malformations.' The vaginal fluid is usually the result of cervical secretions induced by estrogen. In cases of a urogenital sinus, there is a single orifice for the bladder and vagina, and in such cases, the fluid within the vagina may be the result of reflux of urine.' A urogenital sinus may be associated with intersex conditions, one of these syndromes being mixed gonadal dysgenesis (MGD). Presented here is a man with MGD who was found to have hydrocolpos. CASE REPORT An 18-year-oldman with MGD syndrome was referred for an abdominal ultrasound scan. The patient was born with a bifid scrotal sac. He had a hypospadias with the orifice at the base of the penis. A chromosomal workup revealed a karyotype of 45,XO/46,XY. In an explorative laparotomy performed to define the patient's sex, a streak gonad was found on the right side, as well as a tube and a hemiuterus, which were all excised.
