Autonomous ovarian cyst in isosexual precocious pseudopuberty

Autonomous ovarian cyst in isosexual precocious pseudopuberty Gregory W. Baran, MD,* Robert C. Alkema, MD,? Gustav K. Barkett, DO,$ and Kay M. Frisch, RDMS* Precocious pseudopuberty is a syndrome manifested by breast development, maturation of external genitalia, and accelerated growth. Associated anovulatory uterine bleeding most probably results from endometrial proliferation. The term pseudopuberty is given since the clinical manifestations occur in the absence of pituitary gonadotropin stimulation. Autonomous ovarian function is rare as a cause of this syndrome. We present representative sonographic findings of isosexual precocious puberty secondary to the autonomous function of an ovarian cyst. CASE REPORT A white female (5 years, 7 months) with a history of prematurity and mild cerebral palsy complained to her mother of breast tenderness two months prior to admission. The initial physical examination revealed a Tanner stage I11 status with breast development, menarche, and pubic and axillary hair. Plain film radiographs of the sella turcica were normal. Computed tomography (CT) scans of the sella and suprasellar areas were normal. Bone age was advanced to 6 years, 10 months, with no evidence of fibrous dysplasia. Serum DHEAS was 214 ng/mL, which is prepubertal. Serum estrodial was elevatetd at 9.9 ng/dL. Basal serum FSH was less than 1 IU/L and http://www.deepdyve.com/assets/images/DeepDyve-Logo-lg.png Journal of Clinical Ultrasound Wiley
