We describe the successful free tissue transfer of a latissimus dorsi muscle for coverage of a defect over the dorsum of the forearm, hand and metacarpophalangeal joints in a pregnant patient in the second trimester. There are only few reports of microvascular surgery in a pregnant patient. Surgery in the second trimester, although difficult to plan in an emergency situation, has the least risk of anaesthesia to the growing foetus. The physiological considerations in this situation, especially fluid balance, management of hypercoagulability, and use of analgesics, sedatives and antibiotics are all unique in pregnancy and need to be carefully addressed
