Die sakrale Nervenmodulation oder -stimulation (SNM/SNS) ist eine minimal-invasive Methode, die sich als wichtiger Pfeiler in der modernen Therapie der Stuhlinkontinenz erwiesen hat. Mithilfe einer im Bereich der Sakralforamina S2–4 eingebrachten Elektrode erfolgt die Modulation des sakralen Nervenplexus. Hierdurch kommt es über komplexe spinale und supraspinale Rückkoppelungen zu einer verbesserten Rektumcompliance und Kontinenzleistung. Die SNM ist für die Behandlung nahezu aller Arten der Stuhlinkontinenz geeignet. Bei einem erheblichen Anteil der behandelten Patienten kommt es zu einer signifikanten Reduzierung der Inkontinenzepisoden bzw. zu einem kompletten Rückgang der Symptomatik. Auch im Langzeitverlauf lässt sich die Wirksamkeit noch nachweisen. Ein positiver Effekt der SNM in der Behandlung der chronischen Obstipation ist weniger erwiesen, jedoch kann auch sie eine Indikation zur SNM sein. Weitere Indikationsmöglichkeiten zur SNM in der Koloproktologie und der aktuelle Kenntnisstand dazu werden im vorliegenden Beitrag behandelt.
