Mit der Einführung der flächendeckenden Ultraschalluntersuchungen und der Entwicklung neuer Magnetresonanztomographie(MRT)-Techniken wie auch der Verbesserung der Computertomographie(CT)-Diagnostik ist die Radiologie heute in der Lage, viele Fragestellungen der zuweisenden Kollegen zu beantworten. So werden heute die meisten Nierenzellkarzinome im asymtomatischen Stadium durch Ultraschalluntersuchungen entdeckt, was die Prognose und auch die Möglichkeit der organerhaltenden Operation deutlich verbessert. Gleichzeitig erlauben neue Techniken wie die Kontrastmittel(KM)-Dynamik und die Diffusionsbildgebung Aussagen zur Perfusion von den Nieren, um z. B. die Durchblutung wie auch die Funktion der Nieren, auch bei Transplantnieren, zu beurteilen. Diese Verfahren können auch genutzt werden, um Nierentumoren weiter zu differenzieren. Die Diffusionsbildgebung, die ohne Gabe von KM durchgeführt wird, ist v. a. für das nephrologische Patientenklientel von großer Bedeutung, da die NSF(nephrogene systemische Fibrose)-Problematik wie auch die Diskussion um Ablagerung von Gadolinium im Hirn die Gabe von gadoliniumhaltigem KM einschränken. Neue Techniken wie die Diffusion und auch die Perfusionsmessung machen die MRT besonders für das nephrologische Patientenklientel interessant. In diesem Artikel wird die MRT bei der Beurteilung von unterschiedlichen Nierenerkrankungen vorgestellt und auf die Tumordiagnostik ein Schwerpunkt gelegt.
