Microsurgery continues to be considered as a demanding and risky surgical procedure, especially in old people. Moreover, some surgeons are afraid to perform microsurgery in the elderly, because of fear of additional preoperative and postoperative complications. However, in selected cases a microsurgical reconstruction by free tissue transfer is the best reconstructive solution, in order to obtain a good quality of life for those patients. The aim of this paper is to highlight that age itself is not a contraindication for microsurgery, and is not responsible for additional preoperative and postoperative surgical complications. The authors will also indicate that the preoperative American Society of Anesthesiologists (ASA) score and the length of the operation are important indicators for postoperative medical complications.
