Dermatofibrosarcoma protuberans (DFSP) is a spindle cell tumour that is locally aggressive with an infiltrating component but low metastatic potential. It is thought to be associated with previous immunisation sites and also trauma. Since 1964, only five cases have been reported in the literature of DFSP arising in sites with prior immunisation (Coetzee, S African Journal of Surgery 2:49–54, 1964; Green and Heymann, J Am Acad Dermatol 48:S54–S55, 2003; Morman et al. Arch Dermatol 115:1453, 1979; Reed and Wilson-Jones, Arch Dermatol 98:132–135, 1968; Taylor and Helwig, Cancer 15:717–725, 1962). However, there has only been one reported case of a DFSP arising specifically in a bacillus Calmette-Guerin (BCG) vaccination site (McLelland and Chu, Arch Dermatol 124:496–4975, 1988). We report the second case of DFSP arising within a BCG vaccination site. In addition, we review the reported cases of DFSP arising from different types of vaccination scars and discuss the possible pathophysiology of how DFSP may occur.
