Der Hodenhochstand betrifft bis zu 2,7% aller männlichen Neugeborenen. Zur Therapie stehen etablierte hormonelle und operative Verfahren zur Verfügung. Die medikamentöse Therapie mit hCG bietet bei 14–50% der tastbaren und nicht vollständig deszendierten Hoden eine Erfolgsaussicht. Die GnRH-Analoga als hormonelle Therapie der 1. Wahl weisen Erfolgsraten von bis zu 70% auf. Die operative Korrektur des tastbaren Hodens ist in über 90% erfolgreich. Bei einem nicht tastbaren Hoden (intraabdominell) ist nur in ca. 70% eine orthotope, skrotale Platzierung des Hodens zu erreichen. Bei der operativen Korrektur des Bauchhodens kommt es sowohl bei der Autotransplantation als auch bei der Operation nach Fowler-Stephens zu Ischämie und Reperfusion. Diese Zustände könnten weitere Einflüsse auf die Funktion des Hodens nehmen. In der Erfassung der Veränderungen des Hodens unter Ischämie- und Reperfusionsbedingungen könnten weitere Ansätze zur Selektion der optimal anzuwendenden Therapieform des einzelnen Hodenhochstands liegen.
