Clozapine Reactions 1680, p94 - 2 Dec 2017 Neutropenia and leucopenia: 5 case reports In a retrospective cohort study, five men (aged 30-57 years) were described, who developed leucopenia or neutropenia during treatment with clozapine [route, dosage and outcomes not stated]. The men, who had schizophrenia, received treatment with clozapine. The patients developed leucopenia or neutropenia after 34 335 days of clozapine treatment. Out of these five patients, one developed neutropenia, one patient developed both leucopenia and neutropenia (two episodes) and three patients developed leucopenia of which one patient had three leucopenic episodes. At the start of episodes of leucopenia or neutropenia the patients had received clozapine 200 600mg. The patients discontinued clozapine due to leucopenia or neutropenia. However, three patients restarted clozapine after onset of leucopenia or neutropenia. Author comment: "We evaluated results of complete blood count monitoring to compare percentages of patients who developed or would have developed ≥1 hematologic event under the previous and current FDA guidelines in the first year following initiation of clozapine." "Further, clozapine-induced hematologic episodes that warrant discontinuation or interruption of treatment".
