A few authors described extremely rare cases of an accessory scrotum or an accessory labioscrotal fold associated with perineal lipoma in the past. However, our patient had two symmetrical perineal lipomas of similar size in association with absolutely normal male genitalia. The etiology of these abnormal masses is not clear. We performed a complete excision of the lipomas with an elliptical skin excision, leaving a central perineal portion of skin and subcutaneous tissue. The final scar was rotated in such a way as to be placed in the gluteal fold. Gross and microscopic examination of the surgical specimen revealed two lipomas. To our knowledge, this is the first case of such lipomas in this location reported in the literature.
