Aufgrund der invasiven elektrophysiologischen Untersuchungsmethoden und Behandlungsverfahren von tachykarden Herzrhythmusstörungen im Kinders- und Jugendalter haben sich Verständnis und Wissen über die einzelnen Tachykardieformen in den letzten Jahren deutlich verbessert. Der vorliegende Beitrag erläutert die zugrunde liegenden pathophysiologischen Mechanismen mit den zu erwartenden EKG-Veränderungen der vier häufigsten Formen der supraventrikulären Tachykardien bei Kindern und Jugendlichen ohne angeborenen Herzfehler: die atrioventrikuläre Reentrytachykardie auf der Grundlage einer akzessorischen Leitungsbahn, die AV-Knoten-Reentrytachykardie, die fokale atriale Tachykardie und die permanente junktionale Reentrytachykardie. Des Weiteren werden die idiopathischen ventrikulären Tachykardien beschrieben. Inzidenz, klinische Symptomatik, natürlicher Verlauf und Prognose werden für jede Tachykardieform gesondert dargestellt. Auf die medikamentöse und interventionelle Therapie wird in den folgenden Kapiteln dieses Heftes eingegangen. Zusätzlich werden die aktuellen Empfehlungen zur Vorgehensweise bei asymptomatischen Kindern mit einer Präexzitation anhand der zur Verfügung stehenden Leitlinien erläutert.
