Imiquimod Reactions 1704, p193 - 2 Jun 2018 Cutaneous metastatic melanoma: case report A 67-year-old man developed cutaneous metastatic melanoma following treatment with imiquimod [dosage, time to reaction onset and outcome not stated]. The man presented with a two months history of multiple cutaneous deposits, which appeared shortly following application of imiquimod for presumed actinic keratoses to the forearms. Examination revealed a widespread eruption of dark brown, purpuric papules scattered over the trunk, face and lower limbs. Later, he was diagnosed with metastatic melanoma with a positive BRAF V600E mutation. Positron emission tomography scan revealed no definitive visceral metastasis or primary cutaneous melanoma. Author comment: "Paradoxically, there are emerging case reports of the development of keratoacanthomas and infiltrating squamous cell carcinoma at sites of previous imiquimod application."
