It is most commonly seen in individuals with extrahepatic portal hypertension, particularly those with history of gastrointestinal surgery. Various therapeutic modalities for the treatment of bleeding duodenal varices have been reported in the literature. These have included surgery, sclerotherapy, interventional radiology, and endoscopic ligation. Although endoscopic ligation has been established as a very efficacious therapy for bleeding esophageal varices, very few cases have been reported in the literature of successful treatment of bleeding duodenal varices using this modality. We present a case of bleeding duodenal varices in a patient with extrahepatic portal hypertension who was successfully treated with endoscopic ligation. Furthermore, complete obliteration of the duodenal varices was achieved after ligation. We also review different therapeutic modalities for the treatment of gastrointestinal bleeding from duodenal varices. CASE REPORT A 63-year-old white male was admitted with a 24-hr history of melena. The patient had a history of multiple upper gastrointestinal hemorrhages since 1962. At that time
