Key words Lung cancer screening Low dose CT screening National Lung Screening Trial (NLST) Although more people in the USA die each year from lung cancer than from breast, colorectal and prostate cancers combined, patients at risk for developing lung cancer continue to be predominately diagnosed at a late stage, while each of the other cancers benefit from screening and early detection programmes. Some have postulated that lung cancer patients suffer from policy or treatment bias due to the disease prevalence in elderly, lower socioeconomic groups, and minorities who all have less political advocacy, and due to a stigma that lung cancer is largely a self-inflicted disease. Drs Al-Ayoubi and Flores are correct to be cynical about the slow adoption of lung cancer screening in the USA and are understandably frustrated by delays in policy decisions required to approve screening for high-risk patients ( 1 ). However, while the authors are perhaps technically and philosophically correct, the real world of policy development in preventive services required the scientific rigour of the National Lung Screening Trial (NLST) to overcome high hurdles imposed by critics of screening services, and establish a policy that has the broadest and most enduring impact.
