The formula at the bottom of the second column on page 330 was shown as: "# ! 2 k cv  1 DE ¼ 1 þ m  1 q; þ 1 This should be: "# ! 2 k cv  1 DE ¼ 1 þ þ 1 m  1 q as in the reference by Eldridge et al.
