Corrections have been made to Table 2, the results presented in this article. The error affected the Figure 2 and where necessary in the text (page 1931, right calculation of the second spline term and the prediction of column; page 1932, right column) to reflect the correct relative risks in the restricted cubic spline models used for results. These changes do not affect the conclusions of the non-linear dose–response analyses in Table 2 and Figure 2 manuscript, which remain unchanged. V The Author(s) 2018; all rights reserved.
