A 63-year-old man with a history of difficulty in defecating was diagnosed with a retroperitoneal tumor fully occupying the pelvic space. However, extensive histological examination of the resected material revealed only benign prostatic tissue. This is the ninth reported case of ectopic tissue found outside the urinary tract and at a gross weight of 398 g, the largest so far described. The lesion was also of interest because of its strong positive prostate specific antigen.
