The Working Group on ECG-LVH: The annual report 2012
I welcome the opportunity to submit the Annual Report
2012 of the Working Group on Electrocardiographic
Diagnosis of Left Ventricular Hypertrophy (Working
Group ECG-LVH). This Working Group was initiated by
the Journal of Electrocardiology with the aim to update the
conceptual model of the ECG diagnosis of left ventricular
hypertrophy: to shift the interpretation of the ECG beyond
the prediction of increased left ventricular mass, to
understand underlying myocardial alterations leading to the
ECG changes and to predict clinical outcomes.
In the year 2012, the Working Group ECG-LVH
incorporated new members and currently it comprises of
the following 10 members: Ljuba Bacharova (Slovakia, the
Head); Lia Bang (Denmark); Harvey Estes (USA); Joseph
Hill (USA); Peter Macfarlane (UK); Luis Rodriguez-Padial
(Spain); Ian Rowlandson (USA); Giuseppe Schillaci (Italy);
Douglas Schocken (USA); and David Strauss (USA).
Near the end of the previous year, November 28–29,
2011, the 50th Course on Electrocardiography: The
International Workshop, was held in Barcelona, Spain. The
organizers supported the exchange of non-orthodox opinions
of experts, and the following experts were invited to
participate at the session on left ventricular hypertrophy: L.
Bacharova, V. Barrios, V. Bertomeu, JR González Juanatey,
L. Rodriguez Padial, J. Cinca, S. Sclarowsky, P. Macfarlane,
O. Pahlm, P. Tornos. The exchange of opinions and vivid
discussion resulted in three publications in the Journal of
Electrocardiology in 2012.
During the 37th Annual Conference of the International
Society of Computerized Electrocardiology ISCE '12:
Research and Technology Transfer in Computerized
Electrocardiology, in Birmingham, AL, USA, in April
20–24, 2012, a special session on left ventricular
hypertrophy was organized by the Working Group ECG-
LVH: Changing role of ECG in left ventricular hypertro-
phy. The scenario of the session addressed different aspects
on the problem: the updated conceptual model, clinical
experience, current basic research knowledge, computer
simulations as well as eventual practical application into
clinical decision making:
• Estes H: ECG manifestations of left ventricular
• Schillaci G: Electrocardiography vs. echocardiography
in the assessment of LVH: weighing the evidence.
• Hill JA: Structural, functional and metabolic remodel-
ing of myocardium in LVH.
• Bacharova L: Computer simulation of the ECG
manifestations of left ventricular electrical remodeling.
• Strauss D: Differentiation between LBBB and LVH:
Implication for cardiac resynchronization therapy.
As a result, six manuscripts were consequently published
in the Journal of Electrocardiology.
In June 2012, Ljuba Bacharova was invited to deliver a
plenary lecture: New trends in ECG diagnosis of left
ventricular hypertrophy – implications for clinical practice,
at the 20th Annual Meeting of the Alpe-Adria Association of
Cardiology, Bratislava, Slovakia.
At the 39th International Congress on Electrocardiology
ICE 2012, Beijing, China, 9–12 August 2012, Ljuba
Bacharova presented a plenary lecture: Electrical remodel-
ing – a factor that needs to be considered in the ECG
diagnosis of left ventricular hypertrophy.
Two additional manuscripts were published, addressing
clinical implications of the updated conceptual model
proposed by the Working Group ECG-LVH.
The plan for the year 2013 includes:
1. Elaboration of the 3rd Statement of the Working Group
2. Presentations (special sessions on ECG-LVH if
possible) at international meetings. Three possibilities
• The MALT meeting in Noordwijk, The Netherlands,
April 2013 (organized by Cees Swenne),
• The 40th ICE meeting Glasgow, UK, August 7–10,
2013, (organized by Peter W. Macfarlane),
• The Mallorca meeting, November 2013, (organized
by Antonio Bayes de Luna).
3. A clinical study and a cases report by E. Harvey Estes
Jr. et al. on the effect of the rice diet on ECG in patients
with malignant hypertension (analysis of data from
Duke University archive, W. Kempner protocols);
4. A paper by Ljuba Bacharova et al.: The Role of the
ECG for the diagnosis of left ventricular hypertrophy,
invited for the special issue of Current Cardiology
Reviews, edited by Adrian Baranchuk: The value of
the Electrocardiogram for the Diagnosis and Manage-
ment of Cardiac Conditions in the 21st Century;
