•Although MitraClip implantation induces hemodynamic unloading of the left ventricle and atrium, myocardial wall stress does not consistently improve.•Changes in NT-proBNP after MitraClip implantation vary widely.•In patients in whom NT-proBNP decreased after MitraClip implantation, LA volume index and LV and LA longitudinal strain improved.•In patients in whom NT-proBNP increased after MitraClip implantation, LV and LA longitudinal strain worsened, suggesting an increase of myocardial wall stress.
