Extreme Anforderungen von ,,Vollblut" für die Transfusion bereiten jeder Blutbank Schwierigkeiten. Sie sind mit einem hohen Prozentsatz verfallener Konserven verbunden. Es sollte aber noch berücksichtigt werden, daß bei Transfusion einer Vollblutkonserve die Gefahr der Immunisierung gegen Bestandteile, die ein Patient nicht benötigt wie z. B. Granulozyten, Thrombozyten, Plasmaproteine, sehr groß ist. So beträgt der Anteil der ,,Vollblutkonserven" an der Universität Texas, "System Cancer Center M. D. Anderson Hospital and Tumor Institute" weniger als 0,4%. b) Anwendung von Erythrozyten-Konzentraten: Eryth r ozyte n-Konzentrate werden bei allen Routinetransfusionen verwendet wenn der Patient ein Defizit an Erythrozyten hat. Die Anwendung von Erythrozytenkonzentraten hat zur Folge, daß die Zahl der Transfusionsreaktionen zurückgeht. Die Entfernung des größten Teils des Plasmas gestattet es, 0-Konzentrate Patienten mit der Blutgruppe A, B und AB zu transfundieren, wenn Blut der entsprechenden Blutgruppe nicht zur Verfügung steht. Gewaschene Erythrozyten-Konzentrate sind jedesmal dann indiziert wenn die Patienten eine Überempfindlichkeit gegen Leukozyten und/ oder Thrombozyten oder gegen Plasmabestandteile entwickelt haben. c) Anwendung von Thrombozyten-Konzentraten: Diese können entweder direkt aus Frischblut in der Blutbank gewonnen werden oder mittels Thrombozytopherese.
