Methadone Maintenance To the Editor.— Once again, Vincent Dole's wisdom and prescience has been heard (235:217-219,1976). In condemning the bureaucratization of the pharmacologic strategy of methadone maintenance, he has (and I'm sure with much personal sadness) shown how stultifying regulations can pervert a humane treatment concept. Dr Dole has always understood methadone's limitations and hazards. Never had he, nor for that matter, most of the early workers in the field, claimed a panacea was discovered. He points out that the regulations now in force lead to a pseudoprofessionalism; consequently, the zeal of pioneering has been lost, the fear of censure and liability has invidiously arrived, and those of us who worked in the earlier times have despaired and left the field. Pavlov and Skinner taught us that responses can be conditioned by a loop of stimulus and behavior.
