Exercise and Weight Loss in Obese Postmenopausal Women—Reply Exercise and Weight Loss in Obese Postmenopausal Women To the Editor: Dr Irwin and colleagues1 found that regular exercise resulted in reduced body weight and fat among overweight and obese postmenopausal women. We have 2 concerns about these conclusions. First, their sample appeared to be a highly selected group, with as many as 41% having had a higher education. It is possible that their results only apply to their highly motivated sample, and not to overweight and obese postmenopausal women in general. Second, we question the clinical relevance of the amount of weight loss in this study. The authors cite the results of the Diabetes Prevention Program Research Group, which found that a 7% reduction in body weight decreases the incidence of type 2 diabetes. They also refer to the guidelines of the National Heart, Lung, and Blood Institute, which recommend a 5% to 10% weight loss. The authors, however, only found a 1.7% difference in weight loss due to their intervention. Although their results were statistically significant, they seem to be of questionable clinical relevance.
