Black Operculum in Full-Thickness Macular Hole

View LargeDownload A 72-year-old woman had visual loss in the left eye for 3 months. Visual acuity was 20/200 and a full-thickness macular hole with a black operculum was noted. Red-free and fluorescein angiography images showed the change in position of the dark lesion corresponding to movements of the pigmented operculum embedded in the posterior hyaloid. Optical coherence tomography showed a full-thickness macular hole. The patient declined surgical treatment.
