TY - JOUR AU - Obrecht, J.P. AB - Chemotherapy was introduced into the therapy of neoplasia of man a little more than 50 years ago. Since then its importance has increased steadily. More and more substances have been developed. Today, cytostatics are used on various phases of tumor progression. Originally, chemotherapy was employed in the therapy of metastasized tumors once local treatment had failed. Over the last decades, the ongoing development of cancer has brought up new indications, as for example combined chemo- and radiotherapy, adjuvant therapy, and, to allow a more organ- and function-preserving surgery, chemotherapy that is performed before (neoadjuvantly) or with radiotherapy. Chemotherapy is performed as monochemotherapy or, increasingly, as polychemotherapy. Currently, dosis-intensified and high-dose chemotherapy with hematopoietic stem cell support is under clinical investigation. Furthermore, experimentally supported new therapeutic approaches, like antiangiogenesis, have to be proven to be clinically beneficial. Also under investigation is how to circumvent different mechanisms of resistance against cytostatics in a clinical setting with so-called chemosensitizers.
