Abstract: Background: Little is known about cross-national comparisons of the persistence of antihypertensive medication treatment, trends in persistence, and factors associated with persis- tence. The aim of this study was to describe and compare patterns of antihypertensive drug use in a population of elderly patients in the United States (Pennsylvania), Canada (British Columbia), and the Netherlands. Methods: A retrospective cohort study was conducted of Medicare enrollees in a state pharmacy assistance program in Pennsylvania (USA), residents of British Columbia (Canada), and residents of the Netherlands registered in the PHARMO database. Each population included patients 65 years and older who were initiated on blood pressure–lowering treatment between January 1, 1998 and December 31, 2003 and had continuous follow-up for at least 365 days.
