TY - JOUR AU - Kleijn, E. AB - Summary The clinical pharmacy service of a large University Hospital is described. The service consists of a domestic part covering drug distribution, custody, preparation, dispensing, prescription processing and information service and a clinical pharmacological component taking care of drug concentration and dosage management for toxicology and selected therapeutic area. Parts of the service have been implemented in a varying number of clinical departments and wards. Formulary drugs packed in unit of use packages have been implemented on all ward stocks throughout the Hospital. The medication order and administration charting Kardex system works on 80% of the wards. These wards also use the medication cart (trolley). Further implementation is still in progress. The preparation and dispensing routine by pharmacy assistants at the wards before administration of the drugs and charting by nurses is still under study. The clinical pharmacokinetics laboratory serves the reanimation and intensive care ward, the cardiac‐care ward, neurology, internal medicine and anaesthesi‐ology departments and, less frequently, other departments.
