WATSON, MD, San Antonio, Texas A 61-year-old white woman presented to the Otolaryngology Service at Brooke Army Medical Center. Her chief complaint was a longer than 1-year history of progressive hoarseness. She had a 30 pack-year history of tobacco use as well as a history of moderate alcohol consumption. A complete head and neck examination was performed. The neck was negative for adenopathy. A fiberoptic exami- nation of the larynx revealed an exophytic mass along the entire length of the right true vocal cord (TVC) with de- creased mobility of the affected cord. A chest radiograph was normal. The patient consented to direct laryngoscopy with biopsies of the right TVC lesion. The pathologic find- ings were consistent with moderately differentiated squa- mous cell carcinoma (SCC). The lesion was stage T2N0M0. A vertical hemilaryngectomy for SCC of the right TVC was performed. Though contrast CT showed no thyroid cartilage invasion and frozen margins were negative, permanent sec- tions revealed invasion into the thyroid isthmus and thyroid cartilage.
