AB - This article shows two examples of where safety precautions were not exercised. One incident resulted in the death of two men who were killed when a painter's scaffold collapsed inside an elevated storage tank. The other example discussed in this article is a pump that was destroyed when a passing tractor trailer truck flipped the discharge hose up in the air jerking the pump out of the vault.
