TY - JOUR AB - Reactions 1416 - 25 Aug 2012 ★ X S Hydrocodone/hydroxyzine/paroxetine interaction Altered mental status (first report of an interaction between hydrocodone and hydroxyzine): case report A 53-year-old woman developed altered mental status during concomitant use of hydrocodone, hydroxyzine and paroxetine. The woman presented to an emergency department with weakness and altered mental status. Her history included chronic lower back pain, migraines and hypertension. Her medications included hydrocodone 5mg four times daily and paroxetine 20mg daily [routes and durations of treatment to reaction onset not stated]. Five days earlier, she had been prescribed hydroxyzine 25mg every 6 hours as needed [route not stated] for an episode of urticaria. On examination, she was drowsy and confused, with reduced strength in her extremities. She was unable to stand due to weakness and dizziness. Her urine drug screen was positive for opioids, consistent with her prescribed hydrocodone dosage. She was admitted for further assessment, with a diagnosis of altered mental status. All of the woman’s daily medications were withdrawn, and her confusion, drowsiness and dizziness resolved within 36 hours of admission. After a toxicology consult, her hospitalisation was thought to be most likely due to an adverse drug-drug interaction.
