TY - JOUR AB - Reactions 1204 - 31 May 2008 Toxic epidermal necrolysis in an elderly patient?: case report A 77-year-old woman developed toxic epidermal necrolysis shortly after treatment with prednisone [dosage not stated] and Robitussin [specific drugs and dosages not stated]. The woman, who had hypertension, chronic obstructive pulmonary disease (COPD), coronary artery disease and congestive heart failure, received prednisone and Robitussin for COPD exacerbation. Two days later, she presented with a diffuse blistering rash affecting her face and extremities with multiple regions of tenderness. She had a potassium level of 5.7 mEq/L, a blood urea nitrogen level of 32 and a creatinine level of 2 mg/dL. She was admitted to a critical care unit. The woman received the burns protocol. A skin biopsy findings were consistent with toxic epidermal necrolysis. She received methylprednisolone sodium succinate and ciclosporin. Blood cultures detected group B Streptococcus and coagulase-negative Staphylococcus and she received linezolid and ceftazidime. She developed elevated liver enzymes and ciclosporin was discontinued. Her clinical condition improved and her skin lesions resolved after 2 weeks. After 3 weeks’ hospitalisation, she was discharged to a subacute care facility.
