TY - JOUR AB - Reactions 1670, p170 - 23 Sep 2017 Levodopa/carbidopa/ropinirole/ tolterodine Neurogenic orthostatic hypotension, exacerbation of memory problems and daytime sleepiness: case report A 64-year-old man developed neurogenic orthostatic hypotension (nOH) while receiving treatment with levodopa/ carbidopa and ropinirole. Additionally, he experienced exacerbation of memory problems and daytime sleepiness during treatment with tolterodine[route and duration of treatment to reaction onset not stated]. The man with Parkinson disease (PD), presented with fatigue, difficulty in working, along with urinary urgency and frequency. He had been receiving levodopa/carbidopa 100mg/25mg and ropinirole 3mg thrice a day (TID) for PD from seven years. For last six months, he had been experiencing dizziness on standing. Four months previously, his family physician noted a problem of BP drop upon standing, and instructed him to liberalise fluid and salt intake. He had been experiencing fatigue, mild memory issues, and noted slow thought processing and day time sleepiness. His physician had prescribed tolterodine [dosage not stated] for urinary frequency and urgency, which resulted in marked aggravation of his memory issue and day time sleepiness. Three months prior, his movement disorder neurologist reported his BP 98/75mm Hg on seating and 75/60mm Hg on standing with a slight increase in HR.
