TY - JOUR AB - Reactions 1543, p143 - 21 Mar 2015 Toxic epidermal necrolysis and pseudomonas septicaemia: case report A 29-year-old woman developed toxic epidermal necrolysis (TEN) following therapy with ivermectin and methylprednisolone; subsequently, she died to pseudomonas septicaemia. She had a history of DRESS syndrome and Strongyloides infection. She received ivermectin and methylprednisolone [dosages and routes not stated]. Within 1 week, she developed a coalescing maculopapular rash with numerous dusky areas consistent with TEN. She was treated with immune globulin and methylprednisolone sodium succinate [solumedrol], which were ineffective. She was shifted to a burn unit and died to pseudomonas septicaemia. Author comment: "Perhaps corticosteroids and ivermectin modified the cytokine milieu from the eotaxin and IL5 environment associated with DRESS, such that re-exposure caused SJS-TEN, with characteristic keratinocyte cell death through Fas ligand."
