Whilst these results may be consistent with the basic principles of surgical oncology, our paper goes beyond the readily evident for a number of reasons. A meta-analysis was required to ascertain an overall effect of a number of papers, of which approximately one-third had failed to show that positive peritoneal cytology had any significance on patient outcomes in terms of survival or recurrence. In addition, we highlight the discrepancy in yield rates between different methodologies in current usage, and therefore provide impetus for the surgical community to develop a consensus opinion on how best to collect and interrogate such samples. Most importantly, we hope these data inspire rigorous research into how best to treat patients with positive peritoneal lavage—a cohort of patients who may well benefit from intraperitoneal chemotherapy. The clarity afforded by a meta-analysis might, we hope, galvanize a renewed interest in this area. © 2013 British Journal of Surgery Society Ltd.
