(Anaesthesiu 1978;33:950-2) wrote recently describing an apparatus for administering aerosols during ventilation. Their apparatus consisted of a modified bag mount used with a conventional aerosol cannister. We have used conventional metered dose inhalers with their aerosol cannisters attached directly to the circuit by means of a simple sleeve-which is a silicone rubber muff (No. 6421 515 E037E) as used in the paediatric circuit of the Servo 900 ventilator (Siemens Medical Group). The other end of the muff is fitted with a Portex endotracheal adaptor 8 mm (Portex 100/252/080) and plugged into the swivel connector of the circuit (Portex lOO/~S0/00l). The system is shown in exploded form in Fig. 1. The aerosol can be directed straight down the endotracheal tube during ventilation with little turbulence. The system is so simple and inexpensive that it can be available in any operating theatre.
