TY - JOUR AU - Sakakibara,, K. AB - Summary Roux-en-Y gastrojejunostomy was performed to prevent reflux esophagitis after distal subtotal gastrectomy. Subtotal gastrectomy for gastric cancer was performed in 117 cases from 1984 to 1986 (group A) and in 79 from 1987 to 1988 (group B). In group A, first choice reconstruction after subtotal gastrectomy was Billroth I method. Roux-en-Y gastrojejunostomy was performed in patients who showed marked esophageal reflux by pre-operative barium meal examination in a supine position. Of the cases treated with Billroth I, 31.7% developed post-operative reflux esophagitis. In group B, indications of Roux-en-Y procedure were extended to cases with a moderate amount of barium reflux to the esophagus, and also to those in which extensive lymph node dissection, including the bilateral cardiac region was performed. In both groups A and B, no cases of ooux-en-Y anastomosis showed post-operative reflux esophagitis. From the results of gastric acid secretion test, 24-h intra-esophageal pH monitoring, LES pressure study and acid clearance test, it was proved that Roux-en-Y gastrojejunostomy prevented reflux esophagitis by eliminating the flow of alkaline fluid into the stomach remnant. The percentage of Roux-en-Y procedure after subtotal gastrectomy in group B increased, while the incidence of post-operative reflux esophagitis decreased to 7.6%.
