TY - JOUR AU - Rix, T E AB - Sir Latchford et al. have reviewed 10 years experience of the surgical management of desmoid tumours in patients with FAP, with good results for 20 of their 143 patients who required an operation. They have mentioned the 120 patients who did not come to surgery, but they have omitted to include the outcomes for this group. In constructing their algorithm for the treatment of desmoids in FAP, they have included a recommendation to start patients on sulindac and/or tamoxifen, although the case for these drugs in desmoid disease is unproven. A recent article by Hansman et al.1 has shown that 10 of 13 patients had desmoids which did not progress when on this combination of drugs in high doses. Close inspection of this paper (which had no control group) will show how weak the evidence is for using these drugs, which both cause significant side-effects, in desmoid disease. A review from the Cleveland Clinic demonstrated that 10 per cent of desmoids can be expected to resolve spontaneously, with 80 per cent undergoing a cycle of growth and regression without any intervention being required.
