We agree that intraoperative endoscopic sphincterotomy (IOES) requires familiarity with performing endoscopic sphincterotomy (ES) in a supine position, irrespective of whether the procedure is performed by a surgeon or an endoscopist. The availability of resources is a matter of organization, and additional training may be necessary to carry out IOES. We agree that IOES is advantageous over preoperative ES, as shown by the decreased complication rate and shorter hospital stay in our meta-analysis. There is currently no evidence from randomized trials that laparoscopic common bile duct exploration (LCBDE) is better than IOES. Experience in ES is more widely available than experience in LCBDE. It may therefore be easier to adopt the practice of IOES than LCBDE. The cost-effectiveness of IOES versus LCBDE is a topic for further research. We agree on the technical point that laparoscopic cholecystectomy can be completed before ES. Alternatively, the proximal small bowel can be clamped to prevent small bowel distension, as mentioned in the discussion section of our report.
