E-mail: h.moriyama@keio.jp A 69-year-old woman presented with high fever and was diagnosed with possible infectious endocarditis (IE) on the basis of the four positive blood cultures for Streptococcus oralis. She has a history of a mitral valve replacement (Bjo ¨ rk – Shiley) for rheumatic mitral sten- osis. Transthoracic echocardiography (TTE) demonstrated mild perivalvular leakage (PVL) without prosthetic malfunction (Figure 1A) but no vegetation. After 6 weeks of antibiotics therapy, her blood culture was negative.
