TY - JOUR AU - Swartz, Allan, J. AB - Abstract This article reviews some of the reasons outpatients make mistakes in taking their medications and suggests actions that pharmacists may take to remedy this problem. A case study of an illiterate patient who failed to take her medications properly is presented. Pharmacists designed a special patient medication sheet to assist the patient. The aspects of self-medication errors discussed include the inability to comprehend directions, patient attitude toward taking drugs, and unclear directions. Included among the solutions to these problems are supplemental directions, pharmacist-patient consultations, better packaging and patient follow-up. The pharmacist must consider it his responsibility to assure the correct self-administration of drugs by outpatients. This content is only available as a PDF.
