AB - Background: Pure dysgerminomas of the ovary are the most frequent ovarian neoplasms in young women; due to the introduction of effective chemotherapeutic regimens an attempt has been made to preserve fertility for these young patients with dysgerminoma of the ovary. Methods: From 1982 until September 1994 we treated 6 patients with pure dysgerminoma of the ovary by primary surgery preservating fertility in 5 of 6 cases. Primary surgery consisted of unilateral salpingo-oophorectomy, wedge biopsy of the contralateral ovary, omentectomy and lymph node dissection; combination chemotherapy was given to patients with advanced clinical stages. The tumor markers CEA, AFP, β-HCG, CA 125 and lactate dehydrogenase were evaluated prior to surgery and during therapy. Results: At time of diagnosis patients were between 13 and 22 years old; the tumors arose from the right ovary in 5 of 6 cases. 4 patients had stage-I disease, and 2 patients had stage IIC and IIIC, respectively. Unilateral salpingo-oophorectomy was performed in 5 of 6 cases except for one case with known gonadal dysgenesis. AFP, β-HCG, CA 125 and lactate dehydrogenase (LDH) proved to be useful tumor markers in the management of dysgerminomas. One patient had 3 full-term deliveries after initial treatment of a stage-IA disease; 5 patients have been followed up for a mean duration of 95 months (range 51-144 months) with no evidence of disease. Conclusions: Currently, salpingo-oophorectomy is the procedure of choice in young patients with dysgerminoma of the ovary in the absence of involvement of the contralateral ovary and uterus; combination chemotherapy can improve the prognosis in patients with advanced disease. AFP, β-HCG, CA 125 and especially LDH are useful tumor markers in diagnosis, surveillance of therapeutic effectiveness and early detection of recurrent cancer in ovarian dysgerminoma.
