CASE REPORT The patient, a man of 45 years, was first seen in 1960 when he complained of a dry cough which he had had for 10-20 years and, during the previous 4 days, haemo- ptysis which had increased up to a frank haemorrhage of three-quarters of a cupful of blood. He had not had any recent chest infection, he felt well, his appetite was excel- lent, and his weight had been constant for the preceding 10 years. He smoked approximately 20 cigarettes a day.
