Highlights Research Advanced Oropharyngeal Squamous Cell Carcinoma Algorithm Over the past decade, advanced oropharyngeal squamous cell carcinoma has been treated with adjuvant chemoradiotherapy. Advances in surgical technique, along with the recognition of unique characteristics with a more benign clinical course associated with human papillomavirus–positive disease, has led to a resurgence in the use of surgery with adjuvant therapy. Spector and colleagues evaluated the prevalence of various pathological features, including pathologic retropharyngeal adenopathy, to develop an algorithm for deintensification of therapy, which excludes the retropharyngeal site from the treatment volume in an attempt to decrease toxic effects. Invited Commentary Head and Neck National Surgical Quality Improvement Program The American College of Surgeons–National Surgical Quality Improvement Program (ACS-NSQIP) includes more than 500 member institutions representing roughly 10% of US hospitals. Unfortunately, ACS-NSQIP does not collect data on patients undergoing head and neck surgery. Lewis and colleagues describe a process based on the NSQIP model to develop and validate appropriate measures to assess complex head and neck reconstruction surgery. They concluded that their pilot project including 312 patients was able to systematically capture head and neck surgery–specific variables and can be a model for the development of a robust specialty-specific quality improvement program. Guidelines for the perioperative management of patients with OSAS after T&A suggest further research to better define subgroups for whom complications are more likely. Amoils and colleagues reviewed 2 California state databases containing surgery data for more than 100 000 children from 2005 through 2010. They found that younger children and those with multiple comorbidities had higher rates of complications and should be cared for in the hospital setting after T&A. Continuing Medical Education Statistical Model to Predict Common Vestibular Diagnoses Dizziness is a frequent complaint among patients seen by otolaryngologists and often presents with a confusing set of symptoms and physical signs. Previous research developed classification indices from presenting symptoms to aid in diagnosis and treatment. Friedland and colleagues conducted a retrospective review of 414 new vestibular patient intake questionnaires and medical records to identify predictors for the diagnosis of benign paroxysmal positional vertigo, Ménière’s disease, and vestibular migraine.
