Abstract We describe our experience using a deep-plane technique to create an inferiorly based musculocutaneous flap to decrease distal flap necrosis in reconstruction of large anterior cheek and lower eyelid defects. Our early experience with the use of the deep-plane cervicofacial flap has been encouraging. We believe that it deserves more study for use in the population at risk for flap necrosis.
