Abstract Research for more effective and less toxic local anesthetic agents has, over the years, brought to the physician new and valuable agents. One of the most recently introduced is mepivacaine (Carbocaine).* The purpose of this report is to present our initial clinical experience with this new anesthetic agent in a variety of dermatologic surgical procedures. We thought it of interest to include a brief discussion of some of the commonly employed local anesthetics. Brief History and Description of Local Anesthetics The discovery of cocaine by Nieman in 1860 and its first clinical application by Rotter in 1884 are outstanding landmarks. Equally important is the synthesis of procaine by Einhorn in 1905 and the publication of Regional Anesthesia by Labat in 1922. Following the synthesis of procaine, many other local anesthetic drugs have been developed. Procaine hydrochloride (Novocain) milligram for milligram is still one of the least toxic drugs.
