From evidence‐base to practice: implementation of the Nurse Family Partnership programme in England

The aims of this article are to highlight the issues that are relevant to the implementation of a rigorously evidence‐based programme of support, the Nurse Family Partnership programme, into a national system of care. Methods used are semi‐structured interviews with families in receipt of the programme in the first 10 sites, with the nursing staff, with members of the central team guiding the initiative and with other professionals. Analyses of data collected during programme delivery evaluate fidelity of delivery. The results indicate that the programme is perceived in a positive light and take‐up is high, with delivery close to the stated US objectives. Issues pertaining to sustainability are highlighted ‐ in particular, local concerns about cost set against long‐term rather than immediate gains. However, local investment is predominantly strong, with creative methods being planned for the future. Overall, the study shows that within an NHS system of care it is possible to deliver a targeted evidence‐based programme.

Implementation quality of a family-focused preventive intervention in a community-based dissemination

The aims of this study were to determine (1) the degree to which an evidence-based intervention (EBI) delivered outside the context of a research trial remained faithful to the content and design of the programme as intended and as reported in experimental trials of the same programme, and (2) whether implementation quality affected programme outcomes. We report results of an observational study of 11 sites involved in the statewide dissemination of a popular family-focused prevention programme, the Strengthening Families Programme for Parents and Youth 10-14. We found numerous differences between the community-based implementations we observed and researcher-driven implementations of the same programme, but variability in programme delivery and adherence to content were unrelated to programme outcomes. We conclude that short-term outcomes of well-designed EBIs delivered by well-trained facilitators may be robust to minor changes in delivery and content. However, the effects of implementation quality on longer-term outcomes are unknown.

Growing up with a parent with schizophrenia: what children say they need

Despite the rich literature on the effects of parental mental health problems on child development, the needs of children of mentally ill parents have been overlooked in both research and services. This study investigated the needs of a neglected group, namely Chinese adolescent children of parents with schizophrenia, in order to gain insights into the design of programmes for these adolescents. In‐depth interviews were conducted individually with five Chinese adolescent girls whose mother or father was diagnosed with schizophrenia. Analysis of the interview data revealed four common themes: stigma and discrimination; mixed feelings of love and anger; the role of being a carer; and positive gains. The results shed light on the importance of taking cultural context into consideration when providing services for these children and further conducting research in this area. Although mental health problems are regarded as a taboo and associated with shame in Chinese culture, these children, out of a respect for their parents (‘filial piety’1), showed a strong sense of loyalty to their parents and suppressed their anger and sorrow for their parents' sake. Implications for social services for children whose parents have mental health problems and suggestions for future research are discussed.

Deciding best interests: general principles and the cases of Norway and the UK

This article addresses the difficult matter of interpreting the best interest principle, and offers advice for those who must make laws, and those who make decisions within the constraints of those laws. Our approach rests on an assumption that conclusions about best interest are best reached through a reasoned deliberative process. We suggest that legislators should not write substantive assumptions about what is best for every child into their laws; rather, they should indicate a non-exhaustive list of key relevant considerations that decision-makers can review and evaluate in each and every case. Further, the child's own perspective should be imperative in all deliberations about best interest, and a distinction must be made between objective fact and what is invoked as a substantive and contestable assumption. The article supplies a benchmark against which we may review and judge the actual efforts of legislators and decision-makers to determine what is best for any child.
