Examined the coping styles and health behaviors of hypertensive and normotensive patients visiting a primary care setting for acute medical problems. Hypertensive individuals were far more likely to display a “high-monitoring” (information-seeking) mode of coping than normotensive individuals, who tended to be “low monitoring” (information avoiding) in their coping. Although hypertensive patients reported less dysfunction in their current medical problems than did normotensive patients, they nonetheless reported greater concerns about their condition and its impact. Finally, hypertensive patients were rated by physicians as more likely to desire help with both their presenting medical problem and their stress-related problems. Future research should help to specify the exact relations among coping style, stress, and symptom reporting in hypertension.

We first examined relations between psychiatric disorder and coronary heart disease (CHD) in 77 patients presenting with chest pain. The coping profiles of chest pain patients with and without psychiatric disorder and CHD were then compared. Psychiatric patients with no medical illness (n = 129) were also studied. On the basis of previous research we hypothesized specific coping differences across the groups. As expected, chest pain patients without psychiatric disorder scored significantly higher on a problem-focused coping scale than chest pain patients with psychiatric disorder, who in turn scored higher on this scale than psychiatric patients with no medical illness. The opposite pattern occurred for a measure of wishful thinking. Scores of chest pain patients with psychiatric disorder were higher on a measure of avoidance and lower on a measure of seeking of social supports than those without psychiatric disorder. Scores on a self-blame measure were not different across the groups. The results are discussed in the context of illness behavior and somatization.

Rheumatoid arthritis (RA) patients have been identified as a medical population at risk for psychological disorder, largely because of the pain and functional disability that are the hallmarks of the disease. This study examined the degree to which self-reported functional disability and coping efforts contribute to psychological adjustment among adult RA patients over a 6-month period. Adaptive outcomes included maintaining a sense of worth, mastery, and positive affect despite the illness. Hierarchical multiple regression analyses indicated that increases in disability were related to decreased acceptance of illness and increased negative affect. Coping efforts were related to increases in positive affect. The findings provide modest support for the role individual coping efforts play in shaping illness-related outcomes. Although disability is not easily reversed, knowledge about coping strategies that moderate its psychological impact may provide a useful basis for designing psychological interventions to promote adjustment.

Drawing from theory and clinical lore, we consider how individuals are assumed to cope following irrevocable loss. Several assumptions are reviewed reflecting beliefs concerning the grieving process. Specifically, we examine the expectation that depression is inevitable following loss; that distress is necessary, and failure to experience it is indicative of pathology; that it is necessary to “work through” or process a loss; and that recovery and resolution are to be expected following loss. Although limited research has examined these assumptions systematically, available empirical work fails to support and in some cases contradicts them. Implications of our analysis for theoretical development and research are explored. Finally, we maintain that mistaken assumptions held about the process of coping with loss fail to acknowledge the variability that exists in response to loss, and may lead others to respond to those who have endured loss in ways that are unhelpful.

Evaluated the ability of self-regulation and emotional-drive theories to explain effects of an informational intervention entailing descriptions of the experience in concrete objective terms on outcomes of coping with radiation therapy (RT) in men (N = 84) with prostate cancer. The experimental group had significantly less disruption in function during and for 3 months following RT than the comparison group. The intervention had no significant effect on negative mood. Consistent with self-regulation theory, similarity between expectations and experience and degree of understanding of the experience mediated the effect of the intervention on function. Emotional-drive theory was not supported. These results are consistent with prior research with surgical patients and support the relevance of the information-processing explanations of self-regulation theory to coping with stressful experiences associated with physical illness.
