Abstract A total of 2,726 cases of gastric cancer consisting of 670 cases of early cancer and 2,056 cases of advanced cancer were reviewed. Sixty-three out of 2,056 patients with advanced gastric cancer had been examined endoscopically at least twice with an interval of more than six months before surgical operations were performed. Retrospective study of these 63 cases showed that it is not uncommon to find some abnormalities suggesting malignancy in the past endoscopic pictures of patients with advanced gastric cancer. Four demonstrable cases were selected for presentation. The course of these cases seems to support the concept that early gastric cancer as defined by Japanese researchers develops into advanced gastric cancer. Abstract A sensitive and specific radioimmunoassay system has been developed for measuring somatostatin in tissues. Immunization of rabbit with somatostatin yielded the specific anti-somatostatin antibody that did not cross-react with various hormones tested. The radioimmunoassay system could detect as little as 6 pg of somatostatin. The recovery of somatostatin added to tissues was 67.8 ± 3.7%. Inhibition curves of the extracts obtained from a normal pancreas and two gastric carcinomas were parallel to those obtained with synthetic somatostatin. Immunoreactive(IR) somatostatin was not detectable in normal and tumor tissues of the lung and kidney, except in two out of seven lung carcinomas. However, variable amounts of IR-somatostatin (0.3–13.6 ng/g wet wt.) were detected in carcinomas of the pancreas and stomach, although the values were lower than those in respective control tissues. A case of medullary carcinoma of the thyroid showed relatively high levels of serum calcitonin and tissue IR-somatostatin in both primary and metastatic carcinoma. These data suggest that some malignant tumors could produce somatostatin-like substance, but its significance remains to be elucidated. Abstract A study of 25 young adults under the age of 29 treated by surgery for carcinoma of the large intestine was made to compare with 693 older-age group of patients. The incidence of adenocarcinoma of the large intestine in the first thirty years of life was 3.5%. At the time of surgery, 64% of the patients had metastatic disease, and the five-year survival rate for 19 curatively treated cases was 37%. The coincidental second and third carcinoma of the large intestine and the association of familial polyposis coli were significantly frequent in the younger age group. On the other hand, the association of ulcerative colitis or Crohn's disease could not be found in either of the group. The high incidence of mucin-producing tumors in the younger group was also confirmed. The Cumulative No Evidence of Disease (NED) rate after the curative surgery was lowest among patients of the first-three-decade age group and showed; stepwise high value as the decade of age increased. Abstract A streptococcal preparation, OK-432 (Picibanil), was administered to two patients with gastric carcinoma and two with chronic myelogenous leukemia. All the patients revealed an increasing level of serum complements CH50, C4 and C3, as well as Factor B by 40 days of the therapy. Fixation of C4, C3 and Factor B appeared in the gastric carcinoma tissue and on the leukemic cell surface determined by fluorescent antibody technique, indicating that OK-432 might play a role as an immunopotentiator via the complement system. Estrogen Receptor in Malignant and Benign Breast Tumors and Normal Breast Tissues of Japanese PatientsYAMASAKI,, SUSUMU

Abstract Estrogen receptor (E-R) in breast tumors of patients who were operated at Keio University Hospital was examined routinely. During 10 months from May 1976 till March 1977, 65 breast cancer tissues and 35 benign breast tumors were assayed and 47 surrounding tissues which seemed normal macro-scopically was removed from the mastectomied breast and assayed as control. E-R was evaluated from the points of quantity, specificity and affinity. The simple method of assaying E-R with a single point for routine clinical use was described. The positive ratio of E-R in the breast cancer was 55%. The breast cancer tissues obtained from postmenopausal patients had higher level of E-R content than those from the premenopausals. All of the E-R positive cancers; compared with “normal” tissues, had remarkably higher E-R level. The benign breast tumors also had higher E-R content which was significantly different from the “normal” surrounding breast tissues, suggesting that some benign breast tumors may be hormone-dependent. The incidence of E-R positive benign breast tumor and E-R positive “normal” breast tissue in Japanese patients was higher than that in patients in western countries. The dissociation constant (Kd) of the receptor in E-R positive breast cancers ranged from 0.12 × 10−10M to 4.76 × 10−0 [1.07 ± 0.91) × 10−0 (mean ± S.D.)]. The specific E-R had much stronger affinity than other non-specific proteins. There was no correlation between the histological type of tumors and the E-R level. Abstract A phase 1 study of aclacinomycin A was performed in a series of 22 patients with incurable malignant tumors. Aclacinomycin A was injected intravenously at a dose ranging from 15 to 70mg/m2. The toxic reactions encountered were anorexia, nausea, vomiting, leukopenia, and thrombocytopenia, but they were mild and transient. A dose-toxicity relationship was mainly observed in thrombocytopenia and leukocytopenia. Only the transient T wave depression was observed when 40 mg/m2 of aclacinomycin A was given intravenously. Abstract A patient with urinary bladder pheochromocytoma and another pheo-chromocytoma in the para-aortic region is presented. Also, nine cases which have already been reported in Japan are reviewed. The following conclusions are made: bladder pheochromocytoma can appear at any age in either sex. The clinical triad consisting of hypertension, gross intermittent or transient painless hematuria and micturitional attacks typical of pheochromocytoma are seen in most cases. Abstract A 42-year-old woman with leiomyoblastoma of the stomach is described. She is living well for more than five years following operation. The clinical findings of 40 Japanese patients with leiomyoblastoma were also reviewed. In this series, the tumor was essentially seen in adults. The average age of the patients was 53, and the sex ratio was equal. The most important and valuable diagnostic aid was roentogenography. The tumors are usually found in the antrum, but rarely in the cardia. Endoscopically, a polypoid mass, which is frequently umbilicated or ulcerated, can be visualized. The size of tumor varies from 1.2 cm to 11 cm in diameter and the growth usually occurs singly. Abstract Using the small-bore needle developed at Tokyo Medical College Hospital, accurate diagnoses were obtained in 146 out of 182 cases of pulmonary carcinoma (80.2%) from 1967–74. Complications arose in 8.1% of all punctured cases, most of which were slight. There was, however, one case of implantation. Only seven cases of needle tract implantation have so far been reported in the world. Needle biopsy has been described as a valuable diagnostic method in certain cases, particularly in those cases where other procedures fail to yield definitive diagnosis.
